Preocuparea pentru eroarea medicala neintentionata nu este una noua in comunitatea
academica mondiala si româneasca.  Studierea imperfectiunilor diverselor sisteme de
sanatate a condus relativ recent la concluzii ingrijoratoare privind eroarea de diagnostic
si ulterior eroarea medicala neintentionata – ca o consecinta nedorita a proceduralizarii
excesive, a monetizarii brute si a limitarii prin protocol a actului medical care, oricât de
mult s-a dorit a fi “robotizat”, el ramâne la latitudinea imperfectiunii sublime a medicului.

Necesitatea unei schimbari este generata de incindenta tot mai mare a erorilor
de diagnostic (> 10%).  Procesul de diagnostic nu mai trebuie atribuit unui singur
“jucator” (medicul curant) – vorbim despre o activitate in echipa.
Se prefigureaza un model construit in jurul pacientului. 
Modelul echipelor multidisciplinare exista si se extinde; utilitatea acestora este
presupusa,  dar nu este inca demonstrata.  Pentru a vedea cum poate fi
imbunatatit procesul de diagnostic si cel decizional sunt necesare noi studii.

Manifestarile diverselor patologii nu mai respecta sabloane clasice iar inter-
disciplinaritatea este necesara perpetuu in particularitatea cazurilor.
Medicii pneumologi si alergologi au nevoie de cunostinte medicale comune,
aprofundate și consolidate.  Procesul de diagnostic trebuie sa ia in considerare
cele mai subtile aspecte ale ambelor specialitati.

Problema erorilor de diagnostic este urmarita atent si in România.
Grupul de lucru HIS_RO din Iasi, coordonat de prof. univ. dr. Traian Mihaescu,
este la curent cu noutatile din acest domeniu si militeaza pentru imbunatatirea
sistemului de sanatate prin educatie si training.
Echipa ieseana isi propune sa creeze un cadru ce favorizeaza elaborarea de
protocoale si actiuni in vederea abordarii principalelor probleme in siguranta
pacientilor cu care se confrunta sistemul medical.

Patient safety is fundamental to a qualitative medical system.  Although the subject
of medical errors has been in the spotlight for many years, they continue to be
responsible for many deaths worldwide.  Out of these, the highest morbidity is claimed
by diagnostic errors.  Cognition is the key factor responsible for this type of errors.
The aim of this study was to evaluate how medical residents are taught to employ
cognition in the process of clinical reasoning.

Patient safety is an old but always actual concept that involves the management of
all patients during healthcare in order to prevent medical errors and unintended harm.
The need for improvement has been identified and quantified, but the implementation
has suffered multiple aspects, starting with the concept of change and borrowing
process modelling approach from industry.  Healthcare Improvement Science is a
new approach that addresses change from an academic point of view.

necesare obtinerii schimbarii si acordarii unor ingrijiri centrate pe persoana, 
ingrijiri care sa fie oferite in siguranta, in mod eficace (cu rezultatele dorite),
in mod eficient (fara risipa), in mod echitabil si LA TIMP.

Creating a successful and adherent medical team using
Whatsapp (2018)       15.09.2018
   
A team consisting of 1 pulmonologist, 2 thoracic surgeons, and 5 resident doctors
uses Whatsapp messaging when being engaged in different individual activities
(teaching, traveling etc.).  This communication method significantly improved our
organization of administrative issues and successfully delegated tasks within
the team.   When confronted with medical problems, the group shares opinions on
Xrays and CT scans or any other data that needs a team effort (with patient ID
blurred), significantly reducing the diagnostic time and accelerating treatment
decisions.

Interstitial lung diseases misdiagnosis. A Healthcare improvement

The IPF disease awareness acted, in late 2016, as a red flag of focus on ILDs,
in general.   Tracking complex diagnosis diseases, such as ILDs, require analysis of
possible error sources and learning from mistakes made in the past.
ILD diagnosis would be greatly improved If error sentinels would be in place, such as
trained HIS professional that are placing „red flags” based on previous analyses.
There are numerous studies showing the reduced rate of diagnostic error and hospital
unintended harm in healthcare facilities that use HIS educated professionals to track
and prevent errors.

Pulmonary Diseases, we proposed a formula to assess the possibility to identify
the diagnostic errors since Romania lacks connected nation-wide medical records.

The lack of know-how in preventing and reducing diagnostic errors needs to be
corrected with an educational program, which should be implemented particularly
in medical schools.    Doctors should focus on acquiring a sense of investigating
rather than trying to "watch and match".
